Instructions:
Click on the applicable link to be redirected to the online form
Fill out all required fields of the form, click Submit. The form will automatically be emailed to the ASC Secretary.

Instructions:
Save a copy of the form to your computer, phone, or online (e.g. Google Drive)
Fill out the form with the amounts of each literature item requested and contact information
Email the completed form to literature@limestonena.com
Send e-transfer payment to treasurer@limestonena.com for the GRAND TOTAL amount

Use the Jotform Group Literature Order Form
Instructions:
Check the boxes of the literature you would like to order ensuring correct quantities for each
Fill out the Orderer's Info section at the bottom of the form with all the required sections filled out that are marked with a *
Click Submit at bottom of the page to send the completed form to the Literature Chairperson or click Save to save the form as a draft (a link will be emailed to you so the form may be completed later)
Send e-transfer payment to treasurer@limestonena.com for the total amount
